Assessing the current sustainability capacity of your tobacco treatment program (TTP) is an important step for strategically planning to enhance that capacity. The following tools were used to assess the sustainability capacity of Cancer Center Cessation Initiative (C3I) grantees, and you can use these to evaluate your own program as you plan for long-term sustainment.
C3I Sustainability Plan
C3I participants filled out this form to outline plans to sustain their tobacco treatment programs (TTPs) beyond the funded initiative. The form includes guided questions that can be used to assess your center’s strategies, partnerships, available resources, and additional resources needed to sustain and monitor the impact of your TTP.
The Clinical Sustainability Assessment Tool (CSAT)
The Clinical Sustainability Assessment Tool (CSAT) assesses the current sustainability capacity of a program across 7 domains (Malone et al., 2021). Use this tool to help you identify your program’s sustainability strengths and weaknesses to guide strategic planning and adaptation.
CSAT Domains:
- Engaged Staff & Leadership: Having supportive frontline staff and management within the organization
- Engaged External Partners: Having external support and engagement for the practice
- Organizational Readiness: Having the internal support and resources needed to effectively manage the practice
- Workflow Integration: Designing the practice to fit into existing practices and technologies
- Implementation & Training: Using processes that guide the direction, goals, and strategies of the practice
- Monitoring & Evaluation: Assessing the practice to inform planning and document results
- Outcomes & Effectiveness: Understanding and measuring practice outcomes and impact
